Description: At the revisit on 3/2/05, the facility did not provide appropriate treatment and services to residents to increase range of motion and/or to prevent further decrease in range of motion. Physical therapy evaluations identified 37 residents needing range of motion exercises from nursing staff. Seven of those residents were reviewed and staff did not provide range of motion exercises to all seven residents. |

Description: A survey and annual inspection was completed on 3/30/05 and the revisit was completed on 5/24/05. The facility was not in substantial compliance due to uncorrected Class II violations. The facility failed to identify, assess and provide appropriate restorative nursing care to residents based on resident needs. The facility also failed to assess and provide interventions to meet the nutritional needs of residents fed through gastrostomy tubes. |

The facility also failed to store, prepare and distribute foods under sanitary conditions increasing the risk of cross-contamination and food-borne illness. |
